Chiropractic
All chiropractic treatment is preceded by appropriate evaluation / examination. Chiropractic treatment may include mobilization and/or manipulation (adjustments) that draw from a variety of chiropractic and physical therapy techniques. Treatment can be very gentle, traditional hands on approach, or a combination based on patient preferences and response to care.
Yoga Therapy / Physical Rehabilitation
May include a variety of exercises, stretches, and activities. Yoga Therapy and Physical Rehabilitation Services Require an Initial Assessment, Consultation, and/or Examination. Please contact Evolution of Balance if you have questions or if you are hoping to schedule this service without a formal examination.
DOT Bozeman - DOT Physical / Certified Medical Exam
DOT Physicals / Certified Medical exams for employment, CDL drivers, heavy equipment operators, guide services, School Sports Participation and others.
Please bring your Driver's License to your exam and:
-If you take medications, bring your list of prescribed medications with dosages.
-If you have had a cardiovascular incident (i.e. heart attack or stroke) or a neurological condition, please bring a signed form/waiver filled out by your treating doctor/cardiologist/neurologist explaining your adherence to treatments, regular follow up, and statement regarding your ability to perform work duties.
-If you are being treated for Diabetes, please have your treating provider complete the Insulin Treated Diabetes Mellitus Assessment Form (Form MCSA-5870).
-If you have been diagnosed with Sleep Apnea and are prescribed a CPAP device, please bring your use compliance report associated with your device/account with you to your DOT physical.
